Brighton and Hove Healthy Child Programme School Nurse Service – Health Questionnaire

Information for Year 7 Parent/Carers
Brighton and Hove Healthy Child Programme School Nurse Service
School Nurses from Sussex Community NHS Foundation Trust will be visiting all Year 7 classes over the coming year. Part of this session will involve the distribution and completion of a questionnaire to support healthy lifestyles as well as offering support around transition to secondary school. Please see the attached leaflet for further information about the School Nurse service.
It will be the student’s choice to complete the questionnaire and they will be given the option to meet with the School Nurse individually if they wish. Please see the attached questionnaire for your information. They will also be advised of the weekly drop in run by the school nurse and the ChatHealth text advice line, which is also run by the team.
All questionnaires will be collected by the School Nurse at the end of the talk and will be held confidentially. If the student indicates on their questionnaire that they would like to meet with the school nurse, they will be offered an appointment in school.
If the School Nurse and student feel that they would benefit from ongoing support, the School Nurse will contact the parent/carer by telephone with the student’s agreement, and if unsuccessful at contacting parent/carer, a letter will be sent home.
School Nurses always encourage young people to discuss with their parent/carer if they see the School Nurse.
